【目的】精神障害をもつ患者の生きにくさの理解を看護師の感情体験から多層的枠組を作成し検討する。【方法】看護師に半構造化面接を実施した。慢性期病棟、急性期病棟、外来の部署別で逐語録を起こし意味単位で全て抽出し、生きにくさが述べている文脈に着目しコード化する。次に部署ごとにコード化したものが「社会」「組織・状況」「個人」の観点のどこから述べたものかで読み取り、さらに3分類する。分類したコードを類似性に基づき抽象度を上げサブカテゴリ、カテゴリを生成する。【結果】慢性期病棟は【社会的弱者の立場】【長期入院生活の立場】【孤立無援的立場】、急性期病棟は【社会防衛的立場】【行動制限の立場】【只中の危機的立場】、外来は【社会保護的立場】【継続観察的立場】【自己責任論】の多層的枠組を作成した。【結論】枠組検討で患者の生きにくさを見ていくと、多様な場における見解が俯瞰でき理解を深める一助になることが示唆された。 |

【Purpose】Create and examine multilayered framework for understanding difficulties of life of patients with mental disorders based on nurses' emotional experiences.【Method】Semi-structured interviews were conducted with nurses. Verbatim is recorded in each department of chronic care ward, acute care ward, and outpatient department, and all are extracted in semantic units, and context in which difficulty of living is stated is focused on and encoded. Next, codes for each department are read and further classified into three categories according to where they are mentioned from viewpoints of "society", "organization / situation", and "individual". Abstract classified code based on similarity to generate subcategories and categories. 【Results】A multilayered framework was created for chronic care wards with "socially vulnerable standpoints" "long-term hospitalization" "isolation and helplessness" acute care wards with "social defense standpoints" "behavioral restriction positions" "crisis standpoints" and outpatient wards with "social protection standpoints" "continuous observational positions" "self-responsibility theory." 【Conclusion】It was suggested that looking at difficulties of patients' lives in framework study would help to deepen understanding by providing bird's-eye view of views in various settings. |
