わが国は、現在未曽有ともいえる人口の高齢化が進みつつあり、社会構造における様々な変革の必要性が叫ばれるようになってきた。医療制度もその例外でなく、医療そのものの継続性を念頭に、システム(制度)とファイナンス(経済性)の両面からの改変が急務となってきている。 医療の定義を「人の時間的、空間的、精神的活動域の拡大をはかる学問である」(戸田嘉秋神戸大学名誉教授)とすれば、これまでの医療は人の寿命の伸長、つまり時間的活動域の拡大に偏重してきた感は否めない。本来、時間的活動域と、空間的、精神的活動域の拡大は協立・並行するものではなく、時間的活動域の拡大のみに拘泥すれば、運動機能障害やアルツハイマー型認知症などの、空間的・精神的活動の障害の増加に繋がると考えられる。 筆者らが、日常の高齢者診療の中で経験した、適応外の強制栄養を継続される症例や遷延性意識障害症例、褥創症例を提示し、これらの症例を通じて見た日本の高齢者医療の現況と今後のあり方について考察する。 |

The rapid and great progress of modern medicine has enabled to expand the human average life span, which brought all the countries, especially advanced ones including Japan, into being the elderly societies. On the other hand, on facing the burst increased number of the old people, the present geriatric medicine is facing on the edge of being made a novel paradigm shift for the future. "Medicine is a practical science for extending the chronal, spatial, and mental activities of human", which were said by Dr. Yoshiaki Toda, who was the emeritus Professor of Kobe University. On reflections of the modern medicine, especially the geriatrics, based upon his words, it could not be denied that it puts the excessive emphasis on the chronal human activity, not spatial or mental one. It has resulted to the increased spatial and mental disorders, such as locomotive syndrome, sarcopeniasyndrome and post-cerebral infarction cognition impairment, Alzheimer's disease, respectively.In the future vision of the geriatric medicine in Japan, we should adopt the more integrated viewpoints on it and make a directional shift from cure-oriented medicine to care-oriented one. |
